Please bring your updated medical records, if they have not already been sent to us, along with a photo ID at the time of your appointment. If you are coming to us following pediatric care, please include your immunizations in your medical records.

If you cannot make it to your appointment, we ask that you contact us at least 24 hours in advance to cancel or reschedule your visit. Failure to do so could result in a cancellation fee.
Our physicians strive to provide the best experience possible by developing a treatment plan tailored to your individual needs. As part of that effort, we ask that you provide your most current medication information, as well as any documents related to treatment you may have received (e.g., during an emergency room visit or at another practice) and lab and/or radiology test results.
Garden State Medical Group is able to perform a variety of onsite testing, including blood tests and diagnostic imaging (e.g., X-ray, magnetic resonance imaging [MRI]).
Your insurance plan has strict guidelines as to where each test can be performed, and our staff will help you choose the option that best fits your needs. If Garden State Medical Group is an approved site, we will perform the test during your visit or schedule a follow-up appointment to do so.
Once you have completed your exam, you will be directed to see one of our staff members. At this time, we will schedule any necessary follow-up appointments and provide you with any written prescriptions.
In some instances, insurance companies do not reimburse our practice for the cost of the immunization. Therefore, it may be necessary for you to bring a prescription to a pharmacy and have the immunization administered there.
To help provide you with the best care possible, your physician may discuss important aspects of your overall care. This involves preventive health and wellness guidelines, including a discussion of such topics as nutrition, yearly health screenings, weight and healthy lifestyle choices. This focus on comprehensive care will allow us to help you optimize your health.
