Diabetes Prevention and Education Program
Type 1 and type 2 diabetes education, prevention, and management — including medication management and research-based prevention strategies.
The Diabetes Prevention and Education Program at Garden State Medical Group provides comprehensive support for patients at risk for diabetes, those newly diagnosed, and patients who have been managing type 1 or type 2 diabetes for years. Diabetes is one of the most prevalent chronic conditions in the United States, but with the right education and medical support, it can be effectively managed.
For patients with prediabetes or risk factors such as obesity, family history, or a history of gestational diabetes, our program focuses on research-based prevention strategies. These include structured guidance on nutrition, physical activity, weight management, and blood sugar monitoring to help you reduce your risk of progressing to type 2 diabetes.
For patients already living with diabetes, we provide individualized care that includes medication management (oral medications and insulin therapy), blood glucose monitoring education, A1C testing and tracking, and management of diabetes-related complications such as neuropathy, retinopathy, and cardiovascular disease.
Nutritional counseling is a core component of our program. We help you understand how different foods affect your blood sugar, develop meal plans that are both enjoyable and blood sugar-friendly, and set realistic goals for weight management. We also address the emotional and psychological aspects of living with a chronic condition.
Routine screenings are essential for diabetes management. At Garden State Medical Group, we offer in-house blood testing, retinopathy screening, and foot examinations to detect complications early. Our providers coordinate closely with endocrinologists, ophthalmologists, and other specialists when additional care is needed.
